Below are general office policies.  A detailed office policy will be sent to you once you register with our practice via the patient portal

  • All patients must join our patient portal before meeting with a clinician.  Please contact us for more information about the patient portal.

  • All patients must complete a brief phone screening before meeting with a clinician in person.  These appointments are available on Mondays and Fridays at 8:00 a.m. and can be scheduled by clicking the Request Appointment link below.

  • Please contact your insurance provider to inquire about co-payments and deductibles prior to your session; if you do not know your co-payment, a standard $50 co-payment will be charged until your explanation of benefits is received by the practice.  At that point, your account balance will be adjusted accordingly.

  • All patients will be assessed a fee after service is rendered.  Payment must be submitted prior to the next session.  All payments can be made via your patient portal.

  • Please provide at least 24 hours notice prior to canceling a session or you will be billed our full hourly rate.  You can cancel appointments via your patient portal.
