All patients must join our patient portal before meeting with a clinician. Please contact us for more information about the patient portal.
All patients must complete a brief phone screening before meeting with a clinician in person. These appointments are available on Mondays and Fridays at 8:00 a.m. and can be scheduled by clicking the Request Appointment link below.
Please contact your insurance provider to inquire about co-payments and deductibles prior to your session; if you do not know your co-payment, a standard $50 co-payment will be charged until your explanation of benefits is received by the practice. At that point, your account balance will be adjusted accordingly.
All patients will be assessed a fee after service is rendered. Payment must be submitted prior to the next session. All payments can be made via your patient portal.
Please provide at least 24 hours notice prior to canceling a session or you will be billed our full hourly rate. You can cancel appointments via your patient portal.
